Allemand König Graham und Königin Valanice waren froh, ihre Kinder wieder zu haben. Graham meint, es sei an der Zeit, ihnen seinen alten Abenteurerhut zu überreichen. Als er ihn quer durch den Raum wirft, erleidet er einen Herzinfarkt und bricht auf dem Boden zusammen, und er wird ins Bett getragen. Die einzige Möglichkeit, Grahams Gesundheit wiederherzustellen, besteht darin, die magische Frucht im fernen Land Tamir zu holen. Eine schöne Fee namens Genesta bietet Rosella die Möglichkeit, sich in das Land transportieren zu lassen und die Frucht zu finden. Aber wenn sie einmal transportiert wurde, kann sie nicht mehr zurückgeschickt werden, es sei denn, sie hilft der Fee, ihren Talisman wiederzuerlangen, der von der bösen Hexe Lolotte gestohlen wurde.

Anglais King Graham and Queen Valanice were glad to have their children back. Graham thinks that it is time to pass onto them his old adventurers' hat. When he throws it across the room, he suffers a heart attack and collapses on the floor, and he is carried off to bed. The only way that Graham's health can be restored is by retrieving the magic fruit in the faraway land of Tamir. A beautiful fairy called Genesta offers Rosella the chance to be transported to the land, and find the fruit. But once she gets transported, she cannot be sent back unless she helps the fairy regain her talisman that was stolen by the evil witch, Lolotte.

King's Quest IV: The Perils of Rosella is the first game in the series to have a female protagonist. Rosella will meet characters that will either help or hinder her. The player moves Rosella around with arrow keys, and have her perform actions by typing commands. Unlike the previous King's Quest games, which used the AGI engine, this installment uses the new SCI engine with more detailed graphics; the most visible effect is that the game is paused while the player is typing, giving unlimited time to complete even the longest commands. An AGI version of the game, however, was released as well.

Gameplay-wise, the game is similar to the first two installments of the series, with a large interconnected world open for exploration and no time limit imposed. The game is the first in the series to have a day and night cycle; certain actions can be performed only during a specific time.

Espagnol La historia comienza en el mismo punto en el que concluye la tercera parte. Alexander regresa a palacio con su hermana Rosella y conoce a sus padres, el Rey Graham y la Reina Valanice. Graham les dice entonces a sus dos hijos que ya es hora de pasar su clásico sombrero de aventurero a sangre más joven. Lanzará el gorro al aire y aquel de los dos que lo cace al vuelo se quedará con él. Graham lanza el gorro y en ese mismo momento se desvanece, víctima de un ataque al corazón. Su vida se apaga por momentos. Alexander y Valanice se quedan con él, pero Rosella es incapaz de aguantar la emoción y va a la sala del trono a lamentarse.
Entonces el espejo mágico se ilumina, y un hada llamada Genesta le comunica a través de él que si viaja con ella al reino de Tamir, podrá encontrar una fruta mágica que tiene el poder de curar cualquier enfermedad y de prolongar la vida humana. A cambio, tendrá que ayudar a Genesta a recuperar su talismán, pues sin él sus poderes y su vida se extinguirán. Sin el talismán, además, no podrá mandarla de vuelta a Daventry, y Graham morirá en 24 horas.
El talismán está en poder de la malvada bruja Lolotte, por lo que deberá arreglárselas para llegar hasta su castillo. Al hacerlo, será presa por las criaturas de Lolotte, pero será salvada por el horrible hijo de la bruja, Edgard, que le dice a su madre que quiere casarse con ella. Rosella deberá averiguar cómo escapar a esa proposición, librarse de Lolotte, y recuperar el talismán para devolvérselo a Genesta. Y además debe encontrar y hacerse con la fruta mágica. Todo ello en menos de 24 horas.

Français Le Prince Alexander ayant sauvé la Princesse Rosella d'un dragon, le Roi Graham lance en l'air son chapeau par enthousiasme d'avoir retrouvé son fils perdu et d'avoir sa fille sauvée. Mais il est soudain pris d'une crise cardiaque, et se retrouve dans le coma.

Rosella est terriblement affligée par la situation de son père. Une fée lui apparait, et lui apprend que le seul moyen de sauver son père est d'aller retrouver une pomme magique qui pourra le guérir. Rosella part donc à l'aventure.

Italien Il principe Alexander dopo aver salvato la principessa Rosella da un drago, re Graham lancia il cappello in aria entusiasta per aver ritrovato il figlio perduto e aver salvato sua figlia. Ma improvvisamente ha un attacco di cuore e si ritrova in coma.
nnRosella è terribilmente angosciata dalla situazione di suo padre. Una fata gli appare e gli insegna che l'unico modo per salvare suo padre è andare a cercare una mela magica che possa curarlo. Rosella va quindi all'avventura.
