Allemand Das Spiel bietet mit Ägypten und Rom zwei neue Schauplätze, einen neuen Grafikstil sowie zahlreiche Feinde und Waffen aus „The First Encounter“ und „The Second Encounter“. Darüber hinaus führt das Spiel viele neue, exklusive Feinde und Waffen ein. Die Geschichte dreht sich um Sam, der erneut in die Vergangenheit reist. Gleichzeitig trifft eine Gruppe von Menschen, die sich mit alten Zivilisationen befassen, auf neue Kräfte der hybriden Armee von Mental. Sam muss zunächst nach Ägypten und anschließend nach Rom reisen. Dabei muss er sich unterwegs gegen Horden von Feinden zur Wehr setzen. Die Endgegner des Spiels, die jeweils am Ende zweier Schauplätze auftreten, sind zwei ähnlich aussehende, lykanthropähnliche Kommandanten: Sirian Sphinx in Ägypten und Wolfiator in Rom. Nach der Niederlage des Letzteren kehrt Sam Stone siegreich in seine Zeit zurück.

The game introduces many new, exclusive enemies and weapons. The story resolves around Sam going back in time again as a group of people studying ancient civilizations in the Earth's past encounters new forces of Mental's hybrid army. Sam has to return to Egypt first, and then proceed to Rome, battling hordes of enemies on his way. The game's bosses (each one encountered at the end of two settings) are two similar-looking lychantroph-like commanders: Sirian Sphinx (Egypt) and Wolfiator (Rome). With the defeat of the latter, Sam Stone, again victorious, finally returns to his time.

Le joueur incarne Serious Sam dans 12 niveaux où il devra réduire au silence les larbins de Mental grâce à ses 10 armes.
