Allemand Wizardry V: Heart of the Maelstrom führt die Handlung nach Legacy of Llylgamyn fort: Der durch L'kbreths Orb gesicherte Frieden wird erneut erschüttert, als chaotische Energien aus dem Untergrund in die Welt dringen. Diese konzentrieren sich im Maelstrom, einem ausgedehnten Tunnelsystem unter dem Tempel der Weisen in Llylgamyn. Deine sechsköpfige Heldengruppe wird entsandt, um den mystischen Gatekeeper zu finden, der das Chaos bannen kann – doch er wurde von der abtrünnigen Zauberin Sorn gefangen genommen. Auf dem Weg durch acht Dungeon-Ebenen begegnet ihr Priestern, Monstern, magischen Prüfungen und schließlich den vier Card Lords, die nur helfen, wenn sie ihre passenden Spielkarten erhalten. Erst danach ist der Weg zum Kern des Maelstroms frei, wo es zur finalen Konfrontation mit Sorn kommt. Mit ihrer Niederlage wird der Gatekeeper befreit und die Ordnung in Llylgamyn wiederhergestellt.

Anglais Wizardry V: Heart of the Maelstrom continues the story from Legacy of Llylgamyn. The peace brought by L'kbreth's Orb is shattered when chaotic forces begin to rise from beneath the Temple of Sages. These energies concentrate in the Maelstrom, a vast system of tunnels. A party of six adventurers is sent to find the Gatekeeper, the only one capable of sealing the chaos – but he is imprisoned by a rogue sorceress named Sorn. As the party descends through eight levels, they seek help from the Card Lords, appease them by retrieving their suits, and unlock the path to the Heart of the Maelstrom. There, a final battle with Sorn ensues. With her defeat, the Gatekeeper grants the Heart of Abriel, restoring order to Llylgamyn.

Français Après que l'Orbe de L'Kbreth ait été utilisé pour ramener la paix sur les terres de Llylgamyn, une nouvelle menace fait son apparition dans la région. Ces énergies, appelées « Maelstrom » par les habitants locaux, peuvent être scellées par une personne connue sous le nom de Gardien. Malheureusement, ce dernier a été emprisonné par une puissance ou une personne mystérieuse connue sous le nom de SORN. Un groupe d'aventuriers courageux part à la recherche du grand prêtre G'bli Gedook, gardien de l'Orbe de L'Kbreth, qui leur explique qu'ils doivent libérer le Gardien et finalement vaincre le mal à sa source.

Foi publicado em 1988 pela Sir-Tech Software, Inc. para o Commodore 64, Apple II e como um inicializador de PC (usando DOS). Uma porta para SNES e FM Towns foi posteriormente desenvolvida e publicada pela ASCII Entertainment no Japão. Dando continuidade aos eventos de Wizardry III: Legacy of Llylgamyn, Heart of the Maelstrom começa após um período de paz, trazido pelo uso do Orbe de L'Kbreth, ser destruído quando os poderes do caos literalmente começam a emergir no mundo.
