Allemand In Liberty City warten unendlich viele Abenteuer - Doch dieses eine ändert alles. Nach dem Mord an einem Mafia-Boss musste Toni Cipriani, ein ehemaliger Handlanger der Leone-Familie, untertauchen. Jetzt ist er zurück, und es ist an der Zeit, alte Rechnungen zu begleichen.

In den Straßen von Liberty City herrscht Aufruhr. Kriegerische Mafiosi ringen um die Vorherrschaft, und die Stadt droht sich in einer Flut von Korruption, organisiertem Verbrechen, Drogenhandel und gewerkschaftlich organisierten Streiks selbst zu zerstören. Toni kann niemanden trauen, während er versucht, das Chaos in der zerrütteten Unterwelt der Stadt zu beseitigen. Wahnsinnige Killer, verkommene Wirtschaftsbosse, zynische Politiker und seine eigene Mutter stehen ihm bei seinem Vorhaben im Weg, die Stadt unter die Kontrolle der Leones zu bringen. Auf einer Odyssee, die Liberty City in seinen Grundfesten erschüttern wird, muss Toni ums Überleben kämpfen und alles tun, um sich seinen Platz in der Führungsriege der Leone-Familie zu sichern.

En el juego recibe tareas de contactos tales como Salvatore Leone, el jefe de los Leone, familia para la que trabaja Toni, de Donald Love para ayudarlo a ser alcalde, de León McAffrey que le ayudará a deshacerse de los Sindacco y de los Forelli, de Maria Latore la esposa del Don, de una mujer llamada Toshiko que quiere vengarse de su esposo y de 8 Ball, el cual en una misión ayuda a Toni y a Donald a volar Fort Staunton.

Ce jeu d'action propulse le joueur dans la peau de Tony Cipriani pour arpenter les rues d'un New York virtuel abritant une mine de malfrats. Comme dans tout GTA, le but est de remplir les différentes missions confiées par les multiples connaissances que l'on se fait au fil de l'histoire, entre deux vols de voitures.

Toni Cipriani, un tempo fidato saggio della famiglia criminale Leone, è stato costretto a nascondersi dopo aver ucciso un uomo artificiale. Ora è tornato ed è tempo che le cose si sistemino. Le strade di Liberty City sono in subbuglio. I mafiosi in lotta lottano per il controllo mentre la città inizia ad autodistruggersi sotto ondate di corruzione politica, criminalità organizzata, traffico di droga e scioperi sindacali. Non ci si può fidare di nessuno mentre Toni cerca di ripulire il caos del caotico mondo sotterraneo della città. Uomini sicari squilibrati, magnati moralmente depravati, politici cinici e sua madre lo ostacolano mentre Toni cerca di portare la città sotto il controllo di Leone.
